A rural community is working to replace outdated healthcare infrastructure and improve fragmented outpatient services. While demand for care is increasing, the effort is constrained by limited access to capital and a lack of internal development capacity, requiring outside expertise to advance a scalable solution.

High Point structured a $18.5M capital stack blending:

•$7M New Markets Tax Credit (NMTC) allocation (secured)

•Project enables 15,000 new outpatient visits annually
•Launch of new outpatient behavioral health program
